PAYMENT, REFUNDS & DAMAGES 1. REGISTRATION & PAYMENT I understand that my spot is not reserved until payment is received in full. Salt Lake Valley Sports (SLVS) reserves the right to remove any team or player if fees are not paid by the deadline. 2. REFUND POLICY (ALL SALES FINAL) I acknowledge that organizing a league involves significant upfront costs. Therefore, all registration fees are non-refundable once the season schedule has been posted. Before Schedule Release: Refunds are at the sole discretion of SLVS and may be subject to an administrative fee. After Schedule Release: No refunds will be given for any reason, including injury, schedule conflicts, or relocation. Weather: We will make every effort to reschedule games cancelled by weather. However, if facility constraints prevent rescheduling, no partial refunds or credits will be issued. 3. PLAYER LIABILITY FOR DAMAGES I agree that I am financially responsible for any damage to facilities or equipment caused by my negligent or willful actions (e.g., breaking a window, damaging turf). I agree to pay the full cost of repairs as determined by SLVS.
CODE OF CONDUCT I agree to exhibit good sportsmanship at all times. I understand that physical abuse, verbal threats, fighting, and the possession of illegal drugs are strictly prohibited. Violation of these rules may result in immediate ejection or permanent suspension from the league without refund.
MEDIA & PHOTO RELEASE I grant Salt Lake Valley Sports the irrevocable right to use my image, likeness, voice, and video recordings in any marketing, social media, or promotional materials without compensation or approval.
OFFICIAL LIABILITY & INJURY WAIVER In consideration of being allowed to participate in any Salt Lake Valley Sports (SLVS) event or activity, I acknowledge, appreciate, and agree that: 1. RISK OF INJURY The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2. ASSUMPTION OF RISK I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3. COMPLIANCE & SAFETY I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. RELEASE OF LIABILITY I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Salt Lake Valley Sports, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. 5. MEDICAL CONSENT In the event of an emergency where I cannot give consent, I authorize SLVS staff to secure medical treatment for me, including ambulance transportation. I agree that I am solely responsible for all medical costs. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
